People with Long-Term Conditions Invited to Help Shape Services Supporting Them
Cardiff and Vale University Health Board is inviting people living with long-term conditions to contribute to reshaping rehabilitation services.
The Health Board’s ‘Live Well Cardiff and the Vale’ programme is bringing together those with long-term conditions, individuals and organisations who support them and healthcare professionals to discuss how we can work together to support people to live well with a Long-term Condition
A series of meetings enabling all participants to share their views on how services, community organisations and businesses can work better together to meet people’s needs. People across Cardiff and the Vale of Glamorgan are invited to join the conversation.
Emma Cooke, Clinical Director of Allied Health Professionals said: “We want people with long-term conditions throughout Cardiff and the Vale of Glamorgan to Live Well, and we are committed to developing our services and working better with partners to facilitate this.
“Through our Live Well Cardiff and the Vale programme, we are actively listening to the views of service users, healthcare professionals, community organisations and support services so that together we can develop high-quality joined up services that meet everyone’s needs.”
Workshops as part of the Live Well Cardiff and the Vale programme are being held virtually throughout the summer, with sessions taking place on the following dates:

· Wed 3rd August
· Thurs 18th August
· Tues 30th August
· Wed 14th September
· Thurs 29th of September
Whether you are an individual with experience of using services, a healthcare professional, or a member of a community organisation or support services, you are very welcome to attend.
