NHS Wales - Service User Feedback

Thank you for your support in testing a set of questions which have been developed with colleagues in NHS Wales to help us understand patients’ experiences of care. Our testing has shown that we need to adapt some wording in the questionnaire, and we would now like to check the changes we have made. To do this we need as many people as possible to complete this shorter version of the updated questions to check that they are fit for purpose. You can complete the survey thinking about your last healthcare appointment, no matter how long ago it was.

The answers you give will not be passed on to your care team or used to change NHS services. The responses are completely anonymous unless you choose to provide your email address at the end of the questionnaire. We will not pass your contact details on to anyone else or share any identifiable information.
