Job Vacancy - Broker Service – Community Liaison Officer

Age Connects Cardiff and the Vale is looking to recruit the following post to work within C1V hub in Barry Leisure Centre and the Vale Community Resource Service (VCRS) based In Barry Hospital. The role will include supporting older people living in the Vale of Glamorgan.

 Community Liaison Officer – Vale Broker Service – Hours negotiable between 14 and 35 a week, salary £8,262 - £20,655 *   (funded until March 2022)

The post holder will need to work flexibly as part of an integrated team of health and social care professionals in a busy environment, ensuring that information on third sector provision is readily accessible to relevant professionals and to broker service provision to meet client need.

 Knowledge of the range of third sector support across Cardiff & the Vale is essential along with an excellent understanding of the wider health and social care environment and experience of working in a multi-agency setting. Excellent communication skills are essential along with organising your caseload and the ability to keep accurate computer records.
