The way we deliver health and social care services is changing….
The Cardiff and Vale Regional Partnership Board (or ‘RPB’) is made up of partners from health, social care and the third sector. We would like to invite you to have your say in our plans for health and social services.
Our aim is to enable people to live the best lives they can in their homes and communities, so you receive the right support, at the right time, in the right place.
We use our funding to help design and improve services to meet people’s needs. Our current work is looking to improve joined-up care for older people by focussing on a number of important areas:
· Preventative support – helping you stay well through easy access to a range of health and care services which are available to you
· Care and support at home – if you become unwell and need help to get you back on your feet, or if you need access to more long-term support we want to ensure the right support is available
· Discharge support – if you have to go into hospital, we want to ensure the right support is available so you can return home as soon as possible
All of these services revolve around you and so we want to meet with you to find out what services you use and where the gaps are. This will help us to make sure that services are developed and shaped for the future.
If you are over 50 and living in Cardiff or the Vale of Glamorgan we hope you will be able to come to one of our discussions, to meet our team and share your views and experiences:

· Online (Zoom): Monday 29th November – 2.00pm-3.30pm
· Additional dates may be added, depending on demand
